The maximum above is the game data's base figure. Community guides describe fog reaching twice that, which is consistent with something raising it - the Delirium atlas passives below adjust how fog spreads and how deeply it builds.
Grand Mirrors come from Capricious Shards, which is why they only start appearing once that shard class is unlocked. The Simulacrum a fog bank seeds at 100% is its own encounter, and the questline that unlocks all of it is its own page.
Manifested Demons
A Rare or Unique monster inside the fog can manifest a demon, which inhabits its body and uses skills of its own. Four elements, in a stronger form on Unique monsters.

Loathsome Mire
A side area a Deceptive Shard can open off the fog. Level 80. Its floor is flooded, and the liquid flows - following the flow is what leads you through it.
What it pays
Two amulet bases exist only for Delirium, and each trades a modifier slot away. Players report they are the Mire's reward, arriving pre-instilled with two notables - the game data carries the bases and their trade-off, but not where they drop.


Tablets
Can be used in a personal Map Device to add modifiers to a Map. Its modifier pool renders at delirium tablet.
